After Hours and Emergency Care

If you have a medical emegency dial 9-1-1.  If you call the office after hours,  you can leave a message with the answering service which will be paged to the physician that is on-call.  If your call is concerning an appointment or prescription refills please call during normal business hours.  

Prescription Renewals

Please contact your pharmacy with the refill request and your pharmacy will contact the office for authorization and approval.

Financial

Office Appointment:
Patients are expected to pay in full at the time of service.  In some cases payment arrangements can be made prior to your appointment.

All insurance billing is done through MedStat Billing Service.  They can be contacted by dialing 480.985.1093. 

Surgery:
Payment to the surgeon for your surgical procedure must be paid in full prior to your surgery date. 

The hospital has their own payment policies and must be contacted regarding payments made to them for surgical procedures or testing.
